Non-narcotic adjuvants like gabapentinoids help perioperative analgesia by dampening the signals that carry pain. They bind to the α2δ subunit of voltage-gated calcium channels in the central nervous system, which reduces the release of excitatory neurotransmitters. That leads to less transmission of nociceptive input at the spinal cord level and less amplification of pain signals in the brain, effectively blunting central sensitization that often follows surgery. Because some surgical pain has neuropathic-like components, gabapentinoids can help address those elements as part of a multimodal approach. By decreasing overall pain signaling, they can lower the amount of opioids needed to achieve adequate analgesia, reducing opioid-related side effects. It’s important to remember they are adjuncts, not replacements for opioids, and while they offer benefit, they can cause sedation and dizziness, especially when used with other sedatives or at higher doses. They’re used to enhance analgesia without erasing the need to monitor for safety and potential side effects.
